NB: Chimborazo were leading 1-0 when the Uruguayan referee Angel GARCIA VALENTE
    awarded Pichincha a penalty, from which they equalised.  Chimborazo
    walked off in protest, and Pichincha were declared winners; it was then
    decided to let Azuay replace Chimborazo and replay the final with them.
